When I take the satisfaction survey, it tells me I haven't filled in all fields when I have.

Hello Eli, This error has been fixed. Please try the survey again.

The survey requires all questions to be filled... I don't think this is a good idea. This is the second time I have attempted filling it (after reading the above post that this had been rectified), but still had no luck.

Hello Murphree, There was some confusion on my part about Eli's feedback. Yes, all fields are required to be completed, otherwise users will get the error message. On our end, we should have made this explicit. Apologies for the inconvenience.
